About Vital

Vital Rehabilitation is a network of outpatient clinics in the Chicagoland area committed to providing the absolute highest level of quality care. Vital incorporates a unique multidisciplinary approach to rehabilitation utilizing the skills and expertise of physical therapists, occupational therapists, speech pathologists, developmental therapists, individual and family counseling services for both adults and pediatrics.

Founded in 1995 by licensed Physical Therapist, Tom J. Kokocinski, Vital has grown from a small physical therapy operation to a network of 5 outpatient clinics – Chicago, Chicago – Pediatrics, Park Ridge, Schaumburg and Merrionette Park. In addition to the outpatient clinics, Vital also offers home based therapy and holds partnerships with more than 80 home health nursing agencies in Chicago and the surrounding suburbs – Cook, DuPage, Will and Lake Counties. Vital employees more than 80 therapists and administrative staff combined. Vital’s staff speaks English, Polish, Spanish, Russian, German and Slovenian.

Vital Rehabilitation’s flagship adult clinic is located on the northwest side of Chicago in the Portage Park neighborhood, and has expanded into a 10,000 square foot facility. In addition, Vital owns and operates one of the largest free standing pediatric rehabilitation clinics in Chicago and operates the 3rd largest Early Intervention Program in the State of Illinois for children age birth – 3 years old.

Vital Rehabilitation is also a teaching facility for students interested in pursuing a career in rehabilitation and counseling. Vital is currently affiliated with the following universities/colleges programs:
