Is parking available at Vital?

Yes. Convenient parking is available at all of Vital’s locations.

Do I need a prescription from my doctor?

Yes. Each patient needs a current prescription written in the last 30 days from their doctor.

What do I need to bring to my first therapy appointment?

On your initial visit, please bring your current prescription, a driver’s license or state ID, insurance cards, other billing information and a list of medications.

How long is each visit?

Your initial therapy exam will last approximately one hour. Each follow-up visit will take approximately 45 to 60 minutes.

What should I wear to therapy?

All therapy exams are private. Loose-fitting, comfortable clothing is recommended.

Will my insurance cover my therapy?

Most insurance plans cover outpatient therapy. Usually, there is a deductible and/or a co-payment for each visit – If a co-payment is required by the insurance company, Vital will collect the co-pay at each visit. Keep in mind, each patient’s insurance plan is different, so please contact your insurance company to find out what is covered in your plan.

If I don’t have insurance, can I still receive therapy?

Yes. If you do not have insurance, Vital offers convenient payment plans to meet your therapy needs.

How do I get a copy of my medical records?

Signed consent from the patient requesting medical records is required. Allow at least 5 business days to obtain your medical records. Medical records may be subject to a handling charge – for more information call (773) 685-8482.
