Since 1995, Vital Rehabilitation has partnered with home health agencies who specialize in Nursing Services to assist in their expansion of services. Currently, Vital hold partnerships with more than 140 home health agencies in Chicago and the surrounding suburbs – Cook, DuPage, Will and Lake Counties.

Our skilled, experienced team of therapists are multi-lingual in Spanish, English, Polish and Russian. Our home health services offered include:
